Notes - Died in truck accident. At about 4 a.m., Keith was driving his semi on US-20 near Last Chance, Idaho. He struck a calf and verred into a 10-foot-high lava outcropping on the east side of the highway. The cab was torn to pieces and Keith was ejected from the cab. [3]
